When the foot rolls inward, it causes the shin bone to rotate internally. That rotation puts abnormal stress on the knee joint, particularly on the inner (medial) side. Over time, this leads to:
- Medial knee pain — aching or sharp pain on the inside of the knee that worsens with activity
- Patellofemoral syndrome — pain behind or around the kneecap, often called “runner’s knee”
- Meniscus irritation — the cartilage inside the knee absorbs forces it wasn’t designed to handle

The connection between flat feet and back pain is just as direct, even if it’s less obvious. When the foot pronates, the knee rotates inward, which tilts the pelvis forward and out of alignment.
